Preferred Rate Program Qualifications (where applicable)

  • Dedicated Role(s) for Regional Manager, Corporate Operations or Director of Safety who performs periodic audits for safety program compliance
  • Consistent application of the safety program each location
  • TELS Program (or equivalent)
  • Utilization of lift tools (Hoyer Lifts, Sit to Stand Chairs, Slide Sheets)
  • ICONS identifying patient handling status, "No Lift Policies", etc.
  • Lockout/Tagout program in place for maintenance personnel
  • MVR checks

  • Completed Combined Group application
  • Currently valued, first dollar loss runs for the last 5 years (valued within 120 days of proposed effective date)
